Our office hours are from 7:30 a.m. until 5:00 p.m. Monday through Friday. Appointments are available for each provider to help allow ample time to attend to each patientís needs. However, if time does not allow for a scheduled appointment WE ALWAYS WELCOME WALK-INS.

We ask that you make every effort to arrive on time (or a few minutes early) for your appointment. If it is necessary to cancel an appointment, please call the office as soon as possible to reschedule. This will help others to get an earlier appointment.

Our physicians are available 24 hours a day to meet your needs when emergencies occur. If you need urgent care, please go to the nearest hospital or call 911. The emergency room physician will evaluate your medical problem and consult with our on call physician when necessary. If the office is closed, you may call (731) 587-0730 to reach the on-call physician for Martin Medical Center.


We accept most major health plans and coverage. Our physicians accept Medicare assignment, therefore are participating Medicare providers. If you have any questions regarding your coverage or our participation with your plan please feel free to contact our Billing & Collections staff.

Our office will file your charges to your insurance plan. Please keep the billing office informed of any changes in your insurance status. Although we will gladly assist you in processing your claim, payment of the bill is ultimately your responsibility.

If your charges are to be billed to your employer or workersí compensation, please make sure that you inform the registration staff at each visit.

All claims for auto related accidents are to be paid the day of service; we do not get involved with auto claim settlements. Upon your request we will gladly provide you with a copy of your charges and medical records, if needed, so that you may file with your insurance.


In this age of escalating healthcare cost we realize that there are a number of individuals who do not have insurance coverage. In order to help those individuals we have established a Cash Pay Policy. This policy states: If a patient presents to the office with no insurance coverage, and is prepared to pay the entire cost for the days visit, we will give a 20% discount on the entire amount.


Payment is due at time of service; this includes any deductibles or co-pays. We gladly accept cash, personal checks, MasterCard, Visa, and Discover. Now you can pay your bill on-line through our Patient Portal. To access the Portal click on Patient Portal on the left hand side of the screen.


You have the right to privacy and confidentiality regarding your office visits and records.

You have the right to adequate education and counseling regarding your medical condition.

You have the right to have all procedures, risks, benefits and alternatives explained and your questions answered in lay language.

You have the right to have medicationsí effectiveness and possible side effects explained to you.

You have the right to refuse treatment to the extent permitted by law, and the right to receive information on alternatives and consequences of refused treatment.

You have the right to review your medical records and have them explained.


Patients have the responsibility to give honest, accurate and complete medical history information.

Patients have the responsibility to make sure that they understand what the doctor is saying and if not, ask questions for clarification.

Patients have the responsibility to follow their doctorís medical advice and instructions.

Patients should report any significant changes in their health to their doctor.

Patients should keep appointments or cancel in advance because failure to do so prevents other patients from being seen.

Patients have the responsibility to understand their own insurance benefits, coverage, co-payment responsibilities and obtaining referrals and authorizations.


We value you and want you to be satisfied with our service, care and treatment that we provide. If you have any concerns and/or complaints please let us know as soon as possible. You will be given the opportunity to talk with a member of our Administrative staff to discuss the matter. We will strive to immediately address your concern or resolve your complaint. If we are not able to immediately provide you with a response, we will tell you when you can expect to receive a response from us. Please be assured that letting us know your concerns or complaints will not compromise your care in any way but it will allow us to use this information to better serve you in the future. We appreciate your input.
